Summay of notice ⑴ Official in charge of disbursement of the procuring entity : Hirashima Masakuni, the obligating Officer, Director of Worker's Compensation Administration Division, Labour Standards Bureau, Ministry of Health, Labour and Welfare.
⑵ Classification of the services to be procured : 72.
⑶ Subject matter of the contract : Implementation of the General Survey on Working Conditions in 2024, 2025 and 2026, 1 set.
⑷ An observance of a contract period : From 1 September 2023 to 31 March 2026.
⑸ Delivery place : The place specified by the official in charge of disbursement of the procuring entity
⑹ Qualifications for participating in the tendering procedures : Suppliers eligible for participating in the proposed tender are those who shall :
① Not come under Article 10 (excluding Clause 11) of the Law about the reform of the public service by the introduction of the competition
② Not come under Article 70 of the Cabinet Order concerning the Budget, Auditing and Accounting. Furthermore, minors, persons under conservatorship or persons under assistance who have obtained the consent necessary for concluding a contract may be applicable in cases of special reasons within the said clause.
③ Not come under Article 71 of the Cabinet Order concerning the Budget, Auditing and Accounting.
④ Do not go out of Ministry of Health, Labour and Welfare about duties during a period taking a nomination stop.
⑤ Have Grade "A" or "B" in the public tender qualification category of "Offer of services" by Ministry of Health, Labour and Welfare (The common qualification for all ministries and agencies) in the fiscal years of 2022, 2023 and 2024.
⑥ Prove to complete the duties of the matter to list in specifications.
⑦ Do not be a company having the interest with an outside well-informed person or these people appointed as committee of "Meeting on evaluation and improvement of business entrustment to private sectors in Health, Labour and Welfare Statistical Surveys". And do not be a company having the interest with an outside well-informed person or these people who conducts an examination to determine the successful bidder for this work.
⑧ There is no delinquency of corporation tax, consumption tax and local consumption tax.
⑨ For bidders subject to the following insurance system of each item, and it is not in arrears either of insurance premiums (2 insurance year about ⒠ and ⒡) for the last two years of the bid submission deadline of this bid :
⒜ Welfare annuity insurance
⒝ Health insurance (the thing which association of national health insurance has jurisdiction over)
⒞ Seamen's insurance
⒟ National pension
⒠ Worker's compensation insurance
⒡ Unemployment insurance
⑩ Have acquired at least one of the following : Privacy Mark (JIS Q 15001), ISO/IEC 27001 (ISMS International Standard), and JIS Q 27001 (Japanese Industrial Standards).
⑪ Be proved to have no false statement in tendering documents.
⑫ Be proved neither the business condition nor credibility is considerably deteriorating.
⑬ Not received any administrative sanctions for violation of the Laws and Regulations under the jurisdiction Ministry of Labour, Health and Welfare within 1 year. However, this shall not apply in the case where the supplier took corrective actions and received "Notification of withdrawing for the order of suspension of use" after received an order of suspension of use by Labour Standard Inspection Administration, due to violation of the Labour Standard related laws and regulations (*).

⑭ Not be sent the documents to the public prosecutor's office due to violation of the Laws and Regulations under the jurisdiction Ministry of Labour, Health and Welfare and be made this fact public.
⑮ When a Supplier cannot take duties alone, it can participate as Consortium (say composition constructed for the purpose of sharing the duties concerned, and performing it by plural private companies) which can accomplish duties appropriately. In that case, form a consortium by bid documents presentation time and select a representative, and other people shall participate as a member. It is necessary on this occasion that the representative of the consortium concerned and the member comprise a bid participation qualification to establish by ⑭ from ① mentioned above. In addition, the member of the consortium cannot become the member of other consortia or cannot participate alone. In addition, make an agreement (or the documents which are similar to this) about the consortium organization, and submit the consortium concerned by a bid documents presentation time limit.
