(4) Delivery period : From April1, 2017, through March 31, 2018 (5) Delivery place : OsakaHospital, NTT West (6) Qualification forparticipating in the tendering procedures : Any supplier mayparticipate in this tender, with the exception however, of suppliers who fall under any of thefollowing categories : (A1) Suppliers who have beenincapacitated and are not able to enter the agreement in question ; (A2) Suppliers who have beensubject to the commencement of bankruptcy proceedings, and whohave not yet been reinstated ; (A3) Suppliers under corporatearrangement, civil rehabilitation, corporate rehabilitation,bankruptcy, liquidation proceedings, or the equivalent ; (A4) A person prescribed inArticle 32 Clause 1 of the Act on the Prevention of Unjust Acts byOrganized Crime Group Members (Act No. 77 of 1991) ; (A5) Suppliers who have committed one or more of the following actswithin the period stipulated by NTT WEST (in any case, however, notexceeding three years), or those who employ such persons as a proxy, manager, or an employee in another capacity ; (a) Intentionally performed poor or faulty work in themanufacturing of any product, or in the provision of any service, when performing a contract, orcommitted any unfair act with respect to the quality and/orquantity of any product ; (b) Conspired to obtain unfairprofit in making any tender application or entering into orperforming any contract ; (c) Interfered with othersuppliers making a tender application or entering into orperforming a contract ; (d) Interfered with, or refusedor failed to cooperate with NTT WEST personnel when they wereperforming an inspection or supervision duty ; (e) Failed or refused to enterinto or to perform a contract without justifiable reasons ; (f) Made a false report ormisrepresentation in a tender application procedure, or inentering into or performing a contract ; (g) Committed any other unfairact to interfere with competition ; (A6) Suppliers who are deemed tohave gained unfair competitive advantages as a result of theirinvolvement in inspection and/or designing for the procurement inquestion and/or the preparation of technical specifications for theprocurement in question ; (7) Time-limit for tender :Application for tender should be made by completing the prescribedapplication forms and submitting them by 12 : 00 a.m. March 1, 2017(Local Time) to the address in (8).
