| Summay of notice | ⑴ Official in charge of disbursement of the procuring entity : Sohma Daisuke, Director, Administration Department, National Sanatorium Okukomyoen ⑵ Classification of the services to be procured : 22, 31 ⑶ Nature and quantity of the products to be purchased : Whole-body X-ray CT diagnostic system, 1 set ⑷ Delivery period : From the date of contract signing until March 31, 2027 ⑸ Delivery place : National Sanatorium Okukomyoen ⑹ Qualification for participating in the tendering procedures : Suppliers eligible for participating in the proposed tender are those who shall : ① not come under Article 70 of the Cabinet Order concerning the Budget, Auditing and Accounting. Furthermore, minors,PersonunderConservatorshipor Person under Assistance that obtained the consent necessary for concluding a contract may be applicable under cases of special reasons within the said clause ; ② not come under Article 71 of the Cabinet Order concerning the Budget, Auditing and Accounting ; ③ have Grade A, B or C "Sales of products" for participating in tenders by Ministry of Health, Labour and Welfare (Single qualification for every ministry and agency) in Chugoku Region in the fiscal years 2025, 2026, and 2027 ; ④ The following persons are ineligible to participate in the competition : a Those who have included false information in their qualification application or supporting documents. b Those whose business situation or creditworthiness has deteriorated drastically. ⑤ there not being nonpayment about the premium for most recent two years in a deadline for bid book presentation of this bid about the person whom a system to advocate next is applied to ; a welfare annunity insurance b health insurance c seamen's insurance d national pension e worker's compensation insurance f unemployment insurance ⑥ not be suspended from transactions by the request of the officials in charge of contract. ⑦ Those wishing to participate in the bidding must submit a written pledge stating that they are not affiliated with organized crime groups or similar organizations. ⑧ meet the qualification requirements which the Obligating Officer may specify in accordance with Article 73 of the Cabinet Order. ⑨ In the year immediately preceding the deadline for submitting bids for this tender, any person who has been referred to the public prosecutor's office, received administrative sanctions, or received administrative guidance (limited to those made public by administrative agencies) for violating laws and regulations under the jurisdiction of the Ministry of Health, Labour and Welfare will not be likely to jeopardize the fair implementation of this project or the securing of public trust in this project. ⑩ Those wishing to participate in the competition must submit a "Self-Declaration Form" by 12 : 00 PM on October 22, 2026. ⑺ Time-limit for tender : 12 : 00 P.M.
