| Summay of notice | ⑴ Official in charge of disbursement of the procuring entity : Mitsumoto Onaya, Director, National Hospital Organization Kurihama Medical and Addiction Center ⑵ Classification of the services to be procured : 4 ⑶ Nature and quantity of the services to be required : Test Reagent 1 set ⑷ Fulfillment period : From July 1, 2026 through June 30, 2027 ⑸ Fulfillment place : National Hospital Organization Kurihama Medical and Addiction Center ⑹ Qualification for participating in the tendering procedures : Suppliers eligible for participating in the proposed tender are those who shall : ① not come under Article 5 of the Regulation concerning Contract for National Hospital Organization (NHO). Furthermore, minors, Person under Conservatorship or Person under Assistance that obtained the consent necessary for concluding a contract may be applicable under cases of special reasons within the said clause ; ② not come under Article 6 of the Regulation concerning Contract for NHO ; ③ have Grade A, B, C in "Sales of products" for participating in tenders by Ministry of Health, Labour and Welfare (Single qualification for every ministry and agency) in Kanto-Koshinetsu Regi- on ; ④ meet the qualification requirements which the Obligating Officer may specify in acco-rdance with Article 4 of the Regulation ; ⑤ prove that they can deliver the Test reagent, on the date and to the place specified by the Obligating Officer ; ⑺ Time-limit for tender : 17 : 00 June 26, 2026 ⑻ Contact point for the notice : Contract clerk, Planning Section, National Hospital Organization Kurihama Medical and Addiction Center, 5-3-1 Nobi Yokosuka-shi Kanagawa-ken 239-0841, Japan.
