| Summay of notice | ⑴ Official in charge of a disbursement of the procuring entity : Hiroshi Taniguchi, Director of Accounting Department, Self Defense Forces Central Hospital ⑵ Classification of the products to be procured : 4 ⑶ Nature and quantity of the products to be purchased : Yaz Flex combination tablets 250 cases and 6 other items Provided that the quantity is a temporary estimate at the present time, and it is subject to be changed ⑷ Details of this services : Refer to manual for tender and specifications. ⑸ Derivery Period : from 16, April 2026 to 31, March 2027 ⑹ Delivery Place : Self Defense Forces Central Hospital ⑺ Qualifications for participating in the tendering procedures : Suppliers eligible for participating in the proposed tender are those who shall : ① Not come under Article 70 of the Cabinet Order concerning the Budget, Auditing and Accounting. Furthermore, minors, Person under Conservatorship or Person under Assistance that obtained the consent necessary for concluding a contract may be applicable under cases of special reasons within the said clause. ② Not come under Article 71 of the Cabinet Order concerning the Budget, Auditing and Accounting. ③ Have Grade A, Grade B or Grade C on "Sale of Product" in the Kanto and Koushinetsu area in terms of the qualification for participating in tenders by Cabinet Office. (Single quailfication for every ministry and agency) in the fiscal years 2025, 2026 and 2027. ④ Have an authorized license to sell medical and pharmaceutical products as provided by the drugs, cosmetics and medical instruments act as for medicines. ⑤ Not be currently under a suspension of business order as instructed by the Ministry of Defense. ⑥ It should not be parties concerned of the crime syndicate that receives the exclusion request from the administrative divisions police. ⑻ Time limit for tender : 15 : 00, 15 April 2026 in case of by mail ; 9 : 30, 16 April 2026 in case of by hand. ⑼ Contact point for the notice : Go Tabata, Accounting Division, Self Defense Forces Central Hospital, 1-2-24 Ikejiri, Setagaya-ku, Tokyo 154-8532 Japan.
