⑹ Qualification for participating in the tendering procedures : Suppliers eligible for participating in the open tender are those who shall
① Not come under Article 11 of the Central Nippon Expressway Company Limited's regulation concerning the contract procedures
② Have been qualified through the examination of the documents submitted. Submit following documents by time-limit for tender
・Competition Eligibility Confirmation Application Form (Form 1)
・Copy of commercial register
・Most recent financial statements (balance sheet, income statement, statement of changes in shareholders' equity, and notes)
・A copy of the tax payment certificate issued within the past three months (one of the following documents : No.3, No.3-2, or No.3-3)
・Documents detailing the company's history
・Documents describing the actual sales, manufacturing or delivery records of sodium chloride related to item 3⑴⒜(Form 2)
・Documents certifying that 150 tons of sodium chloride can be delivered within 24 hours from the time of notification of delivery instructions to the delivery location specified in Article 3⑴⒝ and a delivery plan (Form 3)
・Document describing the possession or rental status of a stockpile warehouse (even if it is planned) related to item 3⑴⒞(Form 4)
③ Not have capital relationship and/or personnel relationship with other bidders
④ Have not received suspension of registered contractor status or trader after the acceptance of application form for this tender
