| Summay of notice | ⑴ Official in charge of disbursement of the procuring entity : Shuji Kondo, Director, National Hospital Organization Tokushima National Hospital ⑵ Classification of the services to be procured : 22, 31 ⑶ Nature and quantity of the services to be required : Respirator 1 set ⑷ Rental period : From April 1, 2025 through March 31, 2026 ⑸ Rental place : National Hospital Organization Tokushima Medical Center West ⑹ Qualification for participating in the tendering procedures : Suppliers eligible for participating in the proposed tender are those who shall : ① not come under Article 5 of the Regulation concerning Contract for National Hospital Organization (NHO). Furthermore, minors, Person under Conservatorship or Person under Assistance that obtained the consent necessary for concluding a contract may be applicable under cases of special reasons within the said clause ; ② not come under Article 6 of the Regulation concerning Contract for NHO ; ③ have Grade A, B or C in "Service Offer" for participating in tenders by Ministry of Health, Labour and Welfare (Single qualification for every ministry and agency) in Shikoku Region ; ④ prove to have prepared a system to provide rapid response for maintenance of the procured service ; ⑤ prove to have registered with the relevant authorities, in accordance with the Pharmaceutical Affairs Law, to initiate business of selling medical appliances ; ⑥ meet the qualification requirements which the Obligating Officer may specify in accordance with Article 4 of the Regulation ; ⑺ Time-limit for tender : 12 : 00 March 10, 2025 ⑻ Contact point for the notice : Koichi Motonaga, Section Chief, Accounting Division, National Hospital Organization Tokushima Medical Center West, 1354 Shikiji Kamojima-cho Yoshinogawa-shi Tokushima-ken 776-8585, Japan.
