| Summay of notice | ⑴ Official in charge of disbursement of the procuring entity : Sachio Matsushita, Director, National Hospital Organization Kurihama Medical and Addiction Center ⑵ Classification of the services to be procured : 75 ⑶ Nature and quantity of the services to be required : Cleaning service consignment contract ⑷ Fulfillment period : From April 1, 2025 through March 31, 2027 ⑸ Fulfillment place : National Hospital Organization Kurihama Medical and Addiction Center ⑹ Qualification for participating in the tendering procedures : Suppliers eligible for participating in the proposed tender are those who shall : ① not come under Article 5 of the Regulation concerning Contract for National Hospital Organization (NHO). Furthermore, minors, Person under Conservatorship or Person under Assistance that obtained the consent necessary for concluding a contract may be applicable under cases of special reasons within the said clause ; ② not come under Article 6 of the Regulation concerning Contract for NHO ; ③ have Grade A, B, C or D in "Service Offer" for participating in tenders by Ministry of Health, Labour and Welfare (Single qualification for every ministry and agency) in Kanto-Koshinetsu Regi- on ; ④ meet the qualification requirements which the Obligating Officer may specify in accordance with Article 4 of the Regulation ; ⑤ If changes to the hospital's operations during the contract period require revisions to the content of work or changes to the contract, we will be able to accommodate these changes ; ⑥ A medical related service mark certified business ⑦ It is preferable that the company has acquired ISO14001, ISO9001, or ISO27001 certification.
