| Summay of notice | ⑴ Official in charge of disbursement of the procuring entity : Yoshihito Enomoto, Director General, Shikoku Regional Bureau of Health and Welfare. ⑵ Classification of the services to be procured : 24 ⑶ Nature and quantity of the products to be leased : Liquid chromatograph mass spectrometer, 1 set ⑷ Lease period : From 1 June, 2024 through 31 March, 2029 ⑸ Lease place : The place specified by the Official in charge of disbursement of the procuring entity ⑹ Qualifications for participating in the tendering procedures : Suppliers eligible for participating in the proposed tender are those who shall : ① not come under Article 70 of the Cabinet Order concerning the Budget, Auditing and Accounting. Furthermore, minors, Person under Conservatorship or Person under Assistance that obtained the consent necessary for concluding a contract may be applicable under cases of special reasons within the said clause. ② not come under Article 71 of the Cabinet Order concerning the Budget, Auditing and Accounting. ③ are not being suspended from transactions by request of the officials in charge of contract ④ have Grade A, B or C on "offer of services etc (Rental/Lease)" in the Shikoku area in terms of qualification for participating in tenders by Ministry of Health, Labour and Welfare (Single qualification for every ministry and agency) in the fiscal years 2022, 2023 and 2024. ⑤ prove to have no false statement in tendering documents. ⑥ prove neither the business condition nor credibility is deteriorating. ⑦ for bidders subject to the following insurance system of each item, and it is not in arrears either of insurance premiums (2 insurance year about (ⅴ) and (ⅵ)) for the last two years of the bid submission deadline of this bid : (ⅰ) welfare annuity insurance (ⅱ) health insurance (the thing which association of national health insurance has jurisdiction over (ⅲ) seamen's insurance (ⅳ) national pension (ⅴ) worker's compensation insurance (ⅵ) unemployment insurance ⑧ within the past most recent one years in a deadline for bid book, do not receive administrative disposition, or do not be sent to the prosecution by Ministry of Health, Labour and Welfare jurisdiction contravention. ⑨ meet the qualification requirements which the Obligating Officer may specify in accordance with Article 73 of the Cabinet Order. ⑺ Time-limit for tender : 16 : 00, 16 February, 2024 ⑻ Contact point for the notice : General Affairs Division, Shikoku Regional Bureau of Health and Welfare, 3-33, SunPort, Takamatsushi, Kagawa, 760-0019 Japan.
