事務局からの注意事項・免責事項　※必ずご確認ください

■注意事項
1. 本ミッションは現地集合・現地解散型です。集合場所までの往復移動手段（航空券・タクシー等）、飲食代、宿泊代、海外旅行保険等は参加者各自でご手配ください。費用も自己負担となります。
2. 本ミッションは、英語または日本語で実施します（セッションや訪問先によっては翻訳サービスの使用を検討しています）。
3. 本ミッションは原則として、ミッション公式日程の全行程へのご参加をお申込みの条件とします。
4. 訪問先企業や施設等の意向により同意書への署名や同業他社様等の訪問をお断りする場合があります。
5. 本申込書にご記入いただく情報は、ミッション催行時に訪問先ならびに交流相手に参加者リストとして提供します。また、ジェトロの米国投資環境調査ならびに本ミッションの実施・運営、アンケート・成果把握、フォローアップのために利用します。個人情報に関する点につきましてはジェトロ個人情報保護方針に基づいて適切にお取り扱いします。
6. 本事業の成果把握などのためにジェトロが実施するアンケートのご提出に必ずご協力ください。また、本事業における視察等の模様（写真等を含む）は後日、関係機関と共有あるいはジェトロの情報媒体等を通じて公表することがあります。予めご了承ください。
7. 本ミッションの訪問先詳細については、参加者決定後別途ご連絡します。今後変更する場合がございますので、予めご了承ください。
8. 本ミッションの全日程において、感染対策や隔離など米国の連邦政府や州・自治体政府、ジェトロの指定する防疫措置やルールの遵守にご協力いただく事をお申込みの条件とします。
9. 米国では日本と比較して治療費・入院費が大変高額となるケースがあります。不測の事態に備え、全額補償されキャッシュレスで受診することのできる海外旅行傷害保険への加入を強くお勧めします。なお、クレジットカードなどに付帯されている海外旅行傷害保険のみでは全額は補償されない恐れがありますのでご注意ください。
10. ジェトロでは、ミッション期間中に発生した参加者に係る携行品盗難、携行品損害、疾病治療費用などについて一切負担できかねます。
11. ジェトロから正式のご参加登録完了及び催行決定の通知を受け取った後に、航空券等の手配を開始いただくようお願いします。ジェトロからの通知を受け取る前に、航空券等を手配し、結果として参加いただけない事態が発生しても、ジェトロは一切の費用負担を行いません。お申込みの完了をもってご参加を確約するものではありません。
12. 現在、反社会的勢力（反社会的勢力の定義等は、ジェトロの「反社会的勢力への対応に関する規程」）に該当せず、かつ、関係を有しないこと。また将来にわたっても反社会的勢力との関係を持つ意思がないことを条件とします。

■免責事項

1. 本ミッションの開催地である米国は、現時点（2025年11月）で外務省感染症危険情報レベル1となっていますが、本ミッションは参加者の健康・安全の確保について確認がとれた状況下での開催を検討しています。現地情勢等の諸般の事情に鑑み、ジェトロの判断により中止又は延期となる場合やその他の事由により参加できなくなる可能性があります。
2. 本ミッションが中止もしくは延期となった場合等、申込者の責めに帰することのできない事由により参加できなくなった場合においても、一切の損害（航空券代等のキャンセル料を含みますが、これに限られません。）については、ジェトロはこれを負担しません。
3. 本ミッション参加中に体調不良となった場合、ジェトロは最大限のサポートはさせていただきますが、現地での治療などに伴う滞在期間延長またはその他の不測の事態に伴いかかる費用・損失などに関しては一切補償できません。
4. 天災地変、戦乱、暴動、現地の運送、宿泊機関等の旅行サービス提供の中止、官公署の命令その他のジェトロの関与し得ない事由等により本ミッションが中止された場合においても、予約されたホテル、航空券等のキャンセル料その他の経費、損害をジェトロが補填することはしません。

■同意事項
※イベントへの誠実な参加※
1.ジェトロが企画するイベントの工程に基づき、イベントに参加するジェトロの担当事務局の指示に従い、誠実にイベントに参加します。
2.イベントの内容や日程に関する急な変更等が生じる場合も、事業の実施を阻害することなく、他の同行者、訪問先、及びジェトロ事務局に対する適正なマナーに配慮のうえ参加します。
ジェトロ事務局からの指示や注意喚起
ジェトロ事務局が不相応と判断する振る舞いや行動に対し、指示や注意喚起を行う場合、即座にこれらを改善の上、取りやめます。
秘密の保持
1.イベントへの参加に起因又は関連し、書面、電磁的方法、口頭その他、方法の如何を問わず、開示され、又は知りえた他の同行者、訪問先又はジェトロに関する一切の製品、技術、経営及びジェトロ又は開示者が秘密として指定する情報を秘密として保持し、ジェトロ及び他の開示者の事前の承諾なく第三者に開示しません。
2.イベント中に得た資料や撮影した写真・動画を許可なくインターネットに開示又はウェブサイトやSNS上に掲載しません。
禁止行為
以下の各号に記載の行為を他参加者、訪問先およびジェトロ事務局に対して行いません。
1.脅迫、名誉棄損、侮辱、ひどい暴言など「精神的な攻撃」
2.暴行・障害など「身体的な攻撃」
3.事務局の業務上明らかに不要なことや遂行不可能なことの強制、仕事の妨害等の「過大な要求」
4.私的なことに過度に立ち入るなど迷惑行為による「個の侵害」
5.その他、ジェトロ事務局が不相当と判断する行為
違反の場合の措置
1.万が一、本同意書に定める各事項を遵守できない場合、イベントに参加中であるか否かを問わず、ジェトロ事務局より参加の取り止めを指示され、又は、以後の各イベントへの参加が認められなくなることがあることを理解のうえ、当該指示等があった場合にはこれに従います。
2.前項の場合、ジェトロは何らの責めを負うものではなく、ジェトロに対し特段の申し立ては行わないことを確認します。

■Notes
1. This mission will meet and disband on-site. Participants are responsible for their own transportation to and from the meeting point (airfare, taxi, etc.), food and drink, accommodation, and overseas travel insurance. All expenses are at their own expense.
2. This mission will be conducted in English or Japanese (we may consider using translation services depending on the session and destination).
3. As a general rule, participation in the entire official mission itinerary is a condition of application.
4. We may refuse to sign consent forms or visit competitors depending on the wishes of the companies or facilities we visit.
5. The information you provide on this application form will be provided to the destinations and partners as a participant list at the time of the mission. It will also be used for JETRO's U.S. investment environment survey, the implementation and management of this mission, surveys, outcome assessment, and follow-up. Personal information will be handled appropriately in accordance with JETRO's Personal Information Protection Policy.
6. Please be sure to complete the survey conducted by JETRO to assess the project's outcomes. Furthermore, photos and other information from the project's visits may be shared with relevant organizations or published through JETRO's information media at a later date. Thank you for your understanding.
7. Details of the mission's destinations will be announced separately after the participants are selected. Please note that these details may change.
8. As a condition of application, participants agree to comply with all quarantine measures and rules designated by the U.S. federal, state, and local governments, as well as JETRO, including infection control and quarantine measures, throughout the entire mission.
9. Medical and hospital costs in the United States can be significantly higher than in Japan. We strongly recommend purchasing overseas travel insurance, which provides full coverage and cashless medical treatment, to prepare for unforeseen circumstances. Please note that overseas travel insurance provided by credit cards, etc., may not fully cover all expenses.
10. JETRO will not be liable for any theft or damage to participants' belongings, medical expenses, or other incidents that occur during the mission.
11. Please begin making arrangements for airfare and other travel expenses only after receiving notification from JETRO that your participation registration has been completed and the event has been confirmed. If you make airfare and other travel arrangements before receiving notification from JETRO and are consequently unable to attend, JETRO will not cover any expenses. Completing your application does not guarantee your participation.
12. You are not currently a member of or have any relationship with anti-social forces (as defined in JETRO's "Regulations for Dealing with Anti-Social Forces"). You also have no intention of forming any relationship with anti-social forces in the future.

■Disclaimer
1. The United States, where this mission will be held, is currently (as of November 2025) classified as a Ministry of Foreign Affairs Infectious Disease Risk Level 1. However, we are considering holding this mission under circumstances where we can confirm that the health and safety of participants can be ensured. In consideration of various circumstances, including local conditions, JETRO may decide to cancel or postpone the mission, or you may be unable to participate for other reasons.
2. JETRO will not bear any damages (including, but not limited to, cancellation fees for airfare, etc.) if the mission is canceled or postponed or if an applicant is unable to participate due to reasons beyond the applicant's control.
3. If an applicant becomes ill during the mission, JETRO will provide maximum support, but cannot compensate for any expenses or losses incurred due to an extension of stay due to on-site treatment or other unforeseen circumstances.
4. If the mission is canceled due to natural disasters, war, riots, the suspension of travel services by local transportation and accommodation providers, orders from government agencies, or other reasons beyond JETRO's control, JETRO will not compensate for cancellation fees for booked hotels, airfare, etc., or other expenses or damages.

■Agreements
※Honest Participation in the Event※
1. I will faithfully participate in the event in accordance with the schedule of the event planned by JETRO and in accordance with the instructions of the JETRO office in charge of the event.
2. Even if there are sudden changes to the event content or schedule, I will participate without disrupting the implementation of the project and while being considerate of other accompanying persons, visiting parties, and the JETRO office.
Instructions and Warnings from the JETRO Office
If the JETRO office issues instructions or warnings regarding behavior or actions that it deems inappropriate, I will immediately correct and discontinue such behavior.
Confidentiality
1. I will maintain confidentiality of all products, technology, management, and information designated as confidential by JETRO or the disclosing party regarding other accompanying persons, visiting parties, or JETRO that I learn or that arises from or relates to my participation in the event, whether in writing, electronically, orally, or by any other means, and will not disclose such information to any third party without the prior consent of JETRO or the disclosing party.
2. I will not disclose any materials obtained during the event or any photos or videos taken during the event online or post them on websites or social media without permission.
Prohibited Acts
I will not engage in any of the following acts against other participants, parties visited, or the JETRO Secretariat.
1. "Mental attacks" such as threats, defamation, insults, or severe verbal abuse.
2. "Physical attacks" such as assault or injury.
3. "Excessive demands" such as forcing the Secretariat to perform tasks that are clearly unnecessary or impossible to perform, or interfering with work.
4. "Invasion of privacy" through nuisance behavior such as excessive intrusion into private matters.
5. Other acts deemed inappropriate by the JETRO Secretariat.
Measures in the Event of Violation
1. I understand that if I fail to comply with any of the terms of this Agreement, I may be instructed by the JETRO Secretariat to withdraw from my participation or be barred from future events, regardless of whether I am currently participating in the event. I will comply with any such instructions.
2. In the case of the preceding paragraph, JETRO shall bear no responsibility and I confirm that no special claims will be made against JETRO.
