Note: The bid amount must include all costs necessary for the delivery of the purchased
items. For determining the successful bidder, 10 % of the bid amount (with any fraction less
than one yen rounded down) will be added to the bid amount stated in the bid form, and the
total will be considered the bidding price. Therefore, regardless of whether the bidder is a
taxable or tax-exempt entity under the consumption tax law, the amount written in the bid
form must be 100 / 110 of the estimated contract amount.
2. Qualifications for Participation
(1) Must not fall under the provisions of Articles 5 and 6 of the Contract Rules of JCHO.
However, minors, adult wards, or persons under assistance who have obtained the necessary
consent for contract conclusion are considered as exceptions under these rules.
(2) Must have Ministry of Health, Labour and Welfare Competitive Participation
Qualification (All-Government Unified Qualification) for "Sale of Goods" with a grade of A
to D, and must be qualified to participate in the Tokai-Hokuriku area.
(3) Must have the qualifications as specified by the finance officer in accordance with Article
4 of the Contract Rules.

From the date of this announcement until June 20, 2025, at the distribution location listed in
(1), from 9:00 AM to 5:00 PM (excluding Saturdays, Sundays, and national holidays).
For those who cannot come to the hospital, documents will be mailed (postage at
requester's expense; a return envelope such as a Letter Pack must be enclosed). Contact in
advance with ample time.
(3) Deadline for Submission of Qualification Documents:
June 23, 2025, by 5:00 PM (must arrive by this time if sent by mail)
(4) Deadline for Submission of Bid Form:
On the day of the bid opening (must arrive by 5:00 PM on the business day before if sent by
mail)
